Well Child Wellness Visit Incentive

To be eligible to receive a gift card, you must meet all of the following:

• At the time of the well care visit, your child must be enrolled in one of the University of Utah Health Plan’s Individual Marketplace plans as their primary health insurance coverage. Only children enrolled in an Individual Marketplace plan are eligible for the wellness incentive.
• Your child must turn 3 through 17 years of age between January 1st and December 31st, 2024.
• Your child must be seen by an in-network Primary Care Provider (PCP) for an annual well care visit between January 1, 2024, and December 31, 2024.
• We must receive a claim from your child’s provider for this visit. In order to qualify, the claim must meet the following:
  • You must claim your gift card through the member portal by June 30, 2025.
  • The claim must include one of the following billing codes:
    • CPT: 99381-99385, 99391-99395,99461
    • HCPCS: G0438, G0439, S0302
    • ICD10: Z00.121, Z00.129, Z00.2, Z00.3, Z02.5, Z76.2
• If you or someone else paid for a well care visit out of pocket, this visit does not qualify.
• Visits to a provider for urgent or emergent care are not eligible for this wellness incentive. Visits to a provider due to an illness do not qualify for this wellness incentive.
