    Individual Marketplace 2020 Flu Shot Incentive

    A flu shot is an important part of maintaining your health. Receive a $100 Gift Card for each University of Utah Health Plans member who receives their flu shot between August 1 and December 31, 2020. *

    In most situations, you will not have to do anything to receive your gift card. The gift card is automatically mailed to you once we receive the claim. The first round of gift cards will be going out at the end of October or early November 2020.

    Why is University of Utah Health Plans offering this wellness incentive?

    Flu shots are the best way to prevent getting the flu.  Encouraging our members to be vaccinated with the flu vaccine is a preventative measure that will reduce the risk of becoming ill.  University of Utah Health Plans is offering this incentive to increase flu shot vaccination rates and reduce the risk of hospitalizations.

    Who should get a flu shot?

    It is recommended that every person aged 6 months and older should get a flu shot every year. There are only rare situations in which when someone should not get a flu shot. Talk with your doctor for more information.

    Who is eligible to receive a gift card?

    To be eligible to receive a gift card, you must meet the following criteria:

    • You must be currently enrolled in one of University of Utah Health Plans Individual Marketplace plans in order to be eligible to receive a gift card. If you are enrolled in any other University of Utah Health Plan product you are not eligible for this incentive.
    • Members less than 6 months of age are not eligible for this incentive.
    • The flu shot must have been obtained on or between August 1, 2020, and December 31, 2020.
    • If you receive a flu shot, the claim must be submitted to University of Utah Health Plans for payment.
    • To be eligible for this incentive, a claim for the flu shot must be submitted to University of Utah Health Plans by June 30, 2021. Claims received after this date will not be eligible for the incentive.
    • The claim must include one of the following billing Codes:

    What if I already received a flu shot?

    If you’ve already received a flu shot and your health care provider or pharmacy submitted a claim to U of U Health Plans, you will be receiving a gift card beginning early fall 2020.

    What if I paid out-of-pocket for a flu shot?

    If you paid for your flu shot out of pocket, you may submit a paper claim by following the instructions found here

    • To be eligible for this incentive, a claim for the flu shot must be submitted to University of Utah Health Plans by June 30, 2021. Claims received after this date will not be eligible for the incentive.

    • If you no longer have proof of payment for your flu shot, please submit other documentation (doctor’s note, immunization record, etc.) verifying that you received a flu shot between August 1, 2020 and December 31, 2020 to uuhp@hsc.utah.edu .

    What type of gift card will I receive?

    A $100 gift card from Target. The gift card may be used online or in retail stores locally.

    How will I receive my gift card?

    Once U of U Health Plans has received a claim for an eligible flu shot, we will send you a $100 gift card via USPS First Class Letter Mail. The letter will be sent in an envelope from the University of Utah Health Plans, so be sure to watch for any mail sent by us.

    When will I receive my gift card?

    If you have already received your flu shot, you gift card will be mailed you via USPS First Class Postal mail beginning early fall 2020. If you have not yet received your flu shot, we will send out a gift card after the claim for the flu shot has processed.

    What if I lose my gift card or my gift card is lost in the mail?

    University of Utah Health Plans cannot replace lost or stolen gift cards.

    University of Utah Health Plans will send the gift card to the address we have on file. We cannot replace gift cards that are sent to the wrong address or are lost in the mail. If you have recently moved, please contact us to make sure we have your most recent address in our records.

    How many gift cards am I eligible for?

    Each eligible member may receive one gift card.

    Where can I get a flu shot?

    To get a flu shot, make an appointment with a Network Primary Care Provider (PCP).  Find a network provider here.

    Many Network Pharmacies also provide flu shots without an appointment. Find a Network Pharmacy here. Be sure to call the pharmacy first to make sure they offer flu shots. Don’t forget to bring your U of U Health Plans ID card.

    This is a one-time incentive being offered by University of Utah Health Plans.  This incentive is offered by the University of Utah Health Plans and has no affiliation with the University of Utah Hospitals and Clinics or University of Utah Medical Providers.
