Individual Marketplace 2023 Flu Shot Incentive
A flu shot is an important part of maintaining your health. Receive a $50 Gift Card for each University of Utah Health Plans member who receives their flu shot between August 1 and December 31, 2023. *

Why is University of Utah Health Plans offering this wellness incentive?
Flu shots are the best way to prevent getting the flu. Encouraging our members to be vaccinated with the flu vaccine is a preventative measure that will reduce the risk of becoming ill. University of Utah Health Plans is offering this incentive to increase flu shot vaccination rates and reduce the risk of hospitalizations.
Who should get a flu shot?
It is recommended that every person aged 6 months and older should get a flu shot every year. There are only rare situations in which when someone should not get a flu shot. Talk with your doctor for more information.
How to Claim your Gift Card?
After we process the claim for your flu shot, you will receive an email with instructions to select a gift card through our member portal. If you do not have a member portal account, sign up here. You must claim your gift card by March 31, 2024.
Who is eligible to receive a gift card?
To be eligible to receive a gift card, you must meet the following criteria:
- You must be currently enrolled in one of University of Utah Health Plans Individual Marketplace plans in order to be eligible to receive a gift card. If you are enrolled in any other University of Utah Health Plan product you are not eligible for this incentive.
- Members less than 6 months of age are not eligible for this incentive.
- The flu shot must have been obtained on or between August 1, 2023, and December 31, 2023.
- If you receive a flu shot, the claim must be submitted to University of Utah Health Plans for payment by February 29, 2024.
- The claim must include one of the following billing Codes:

What type of gift card will I receive?
A $50 electronic gift card from Amazon.
What if I lose my gift card?
University of Utah Health Plans cannot replace lost or stolen gift cards.
How many gift cards am I eligible for?
Each eligible member may receive one gift card.
Where can I get a flu shot?
To get a flu shot, make an appointment with a Network Primary Care Provider (PCP). Find a network provider here.
Many Network Pharmacies also provide flu shots without an appointment. Find a Network Pharmacy here. Be sure to call the pharmacy first to make sure they offer flu shots. Don’t forget to bring your U of U Health Plans ID card.

This is a one-time incentive being offered by University of Utah Health Plans. This incentive is offered by the University of Utah Health Plans and has no affiliation with the University of Utah Hospitals and Clinics or University of Utah Medical Providers.
