Breast Cancer Screening Incentive

Why is the University of Utah Health Plans offering this wellness incentive? According to the American Cancer Society, nearly one in every eight women will be diagnosed with breast cancer in their lifetime. Fortunately, breast cancer deaths are declining due to early detection strategies and improved treatments. It is recommended that women age 40 and older receive a screening mammogram every year.

How to Claim your Gift Card? After we process the claim for your annual preventive exam, you will receive an email with instructions to select a gift card through our member portal. If you do not have a member portal account, sign up here.
Who is eligible to receive a gift card?

To be eligible to receive a gift card, you must meet all of the following:

  • At the time of the annual screening mammography, you must be enrolled in University of Utah Health Plan’s Individual Marketplace plan as primary health insurance coverage. Only members enrolled in this plan at the time of service are eligible for the wellness incentive.
  • Women age 40 through 74 years of age at the time of the screening mammography are eligible for the incentive.
  • Your screening mammogram must be completed by an in-network provider between January 1, 2024, and December 31, 2024.
    • We must claim your gift card through our member portal by June 30, 2025.
    • The claim must include one of the following billing codes:
      • CPT: 77061, 77062, 77063, 77065, 77066, 77067
  • If you or someone else paid for the mammogram out of pocket, it does not qualify for the incentive.
What type of gift card will I receive? For annual screening received in 2024: A $50 Amazon.com eGift Card.
For annual screening received in 2023: A $25 Amazon.com eGift Card.
What if I lose my gift card?  University of Utah Health Plans cannot replace lost or stolen gift cards.
How many gift cards can I receive? Only one gift card per member, even if you have more than one mammogram during the year.
What else should I know about this wellness incentive? Toll-FreeThis wellness incentive is a one-time incentive offered by the University of Utah Health Plans. This wellness incentive is offered by the University of Utah Health Plans and has no affiliation with University of Utah Health, University of Utah Health’s providers, the University of Utah Medical Group or University of Utah.
