    Why is University of Utah Health Plans offering this wellness incentive? Annual preventive exams are an important step in keeping yourself healthy. It is important for you to visit a doctor even when you are not sick to help catch medical problems early on.
    • Preventive care is important to achieving and maintaining your best health.
    • Preventive care allows for early intervention and better management of conditions such as diabetes, heart failure, high blood pressure and mental health concerns.
    • Preventive care can extend your lifespan.
    • Preventive care can help avoid costly medical expenses.

    How to Claim your Gift Card? After we process the claim for your annual preventive exam, you will receive an email with instructions to select a gift card through our member portal. If you do not have a member portal account, sign up here.
    Who is eligible to receive a gift card? To be eligible to receive a gift card, you must meet all of the following:
    • At the time of the annual preventive exam, you must be enrolled in University of Utah Health Plan’s Individual Marketplace plan as primary health insurance coverage. Only members enrolled in this plan are eligible for the wellness incentive.
    • You must be 18 years of age or above at the time of the exam.
    • You must be seen by an in-network Primary Care Provider (PCP) for an annual preventive exam between January 1, 2023 and December 31, 2023.
    • We must receive a claim from your provider for this visit. In order to qualify, the claim must meet the following:
      • We must receive a claim from your provider for this visit by March 31, 2024. Claims received after this date will not qualify.
      • The claim must include one of the following billing codes:
        • CPT: 99385-99387, 99395-99397
    • If you or someone else paid for the preventive exam out of pocket, this visit does not qualify.
    • Visits to a provider for urgent or emergent care are not eligible for this wellness incentive. Visits to a provider due to an illness do not qualify for this wellness incentive.
    What type of gift card will I receive? A $50 electronic gift card for a major online retailer.
    What if I lose my gift card? University of Utah Health Plans cannot replace lost or stolen gift cards.
    How many gift cards can I receive? Only one gift card per member, even if you are seen for more than one the preventive exam during 2023.
    What else should I know about this wellness incentive? This wellness incentive is a one-time incentive offered by the University of Utah Health Plans. This wellness incentive is offered by the University of Utah Health Plans and has no affiliation with University of Utah Health, University of Utah Health’s providers, the University of Utah Medical Group or University of Utah.
