How to Find Your Network Participation and Navigate the Provider Directory
University of Utah Health Plans Provider Networks:
Healthy U – A Medicaid physical health network available to eligible Medicaid members throughout the entire state of Utah.
Healthy U Behavioral – A Medicaid mental health and substance use network available to eligible Medicaid members in Davis, Salt Lake, Utah, and Weber counties. This network of behavioral health providers and organizations is combined with the Healthy U network to create Integrated Healthy U. This network is also available to eligible Medicaid members in Summit County.
Healthy U CHIP – A statewide provider network to support the Children’s Health Insurance Program, a state-sponsored physical and behavioral health plan for children not covered by other health insurance.
Healthy Preferred – A provider network for employer groups along the Wasatch Front from Cache to Utah counties and Summit to Tooele counties.
Healthy Premier – A provider network for employer groups in Utah and Southeastern Idaho. It is also available on the Utah Individual Marketplace Exchange.
U Health Plus – A narrow commercial provider network offered within Salt Lake and Davis counties.
Step 1: Visit the Main Website (Public Search)
- Go to uhealthplan.utah.edu.
- Click "Find a Doctor" in the upper right-hand corner.
- Select a provider network before starting your search.
Step 2: Use Filters to Narrow Your Search
To find clinic network participation:
- Select "Clinics" under filters, scroll down, and click "Search Providers."
- Use the Medical Group field to search for your practice or clinic name.
- Scroll down and click "Search" again.
- Locate your clinic in the results and double-click the practice name.
- In the additional information section, you will see the networks your clinic is contracted with.
To Find Facility-Based Provider Networks
- Select "Other Providers" under filters, scroll down, and click "Search Providers."
This includes Home Health Agencies, Durable Medical Equipment providers, and Residential Treatment Facilities. - Enter the facility name to refine your search.
- Scroll down and click "Search" again.
- Locate your facility in the results and double-click the name.
- In the additional information section, you will see the networks your facility is contracted with.
