Healthy Advantage Plus HMO is a Health Plan with a Medicare contract. Enrollment in Healthy Advantage Plus HMO depends on contract renewal.

Healthy Advantage HMO SNP is a health plan with a Medicare contract and a contract with the state Medicaid program. Enrollment in Healthy Advantage HMO SNP depends on contract renewal.

You must continue to pay your Medicare Part B premium.

As a Healthy Advantage HMO SNP member, you must continue to pay your Medicare Part B premium. As a full dual member, your State may cover your Part B premium, based upon your level of Medicaid eligibility.

This plan is available to anyone who has both Medical Assistance from the state and Medicare.

Premiums, co-pays, co-insurance, and deductibles may vary based on the level of extra help you receive. Please contact the plan for further details.
