Appeal Process for Healthy U Members
To File an Appeal
You, your legally authorized representative or your provider may file your appeal. If you need help filing your appeal, call us at 801-587-6480, option 1. Si necesita esta carta en Espaņol, por favor llamenos al 801-587-6480 o 1-888-271-5870 opcion 1.
- You may file your appeal by calling us at 801-587-6480 or 1-888-271-5870 and pressing option 1.
- If you call us to file your appeal, you must also send us a written appeal within 5 business days of your call. If you do not send the follow-up written request within 5 business days of your call, you lose the right to appeal.
- If you do not want to call first, just send us your written appeal. Please use the Appeal Request Form.
- Send the complete written appeal to:
- If you would like to appear in person, make sure you tell us in your written appeal.
- If your provider files your appeal, he or she must have your consent to do so.

How long will it take for a decision to be made?
- Once we get the complete written appeal, we plan to make a decision in 30 calendar days. Sometimes we may need more information. If so, we may take an additional 14 calendar days to make our decision. If we need to take extra time, we will send you a letter.
- If you or your provider believes your life or immediate health is in danger, you may ask for an expedited (quick) appeal by calling Case Management at 801-587-6480, option 2.
- If we agree the decision needs to be made quickly, we will make a decision in 3 working days. If we need more time to make the decision, we can take up to another 14 calendar days. If we need more time, we will send you a letter telling you why.
