University Health Plans is proud to announce it is now in the Medicare market. We have provided healthcare services to Medicaid beneficiaries since 1997 and are now expanding our commitment to serve those who are eligible for Medicare.
Healthy Advantage (HMO) is a Medicare Advantage Prescription Drug Special Needs Plan for people with Medicare and full Medicaid. You are eligible for Healthy Advantage (HMO) if you:
- Are enrolled in Medicare Part A
- Are enrolled in Medicare Part B
- Are enrolled in Medicaid
- Reside in the Healthy Advantage (HMO) service area (Davis, Salt Lake, Utah and Weber Counties)
- Enrollment form - Please complete this form to enroll in the plan. If you need help in completing the form, please contact Healthy Advantage (HMO) at 1-866-472-9479 (TDD/TTY: 1-800-346-4128) from 8 a.m. to 8 p.m. (local time) Monday – Sunday. You may enroll in Healthy Advantage (HMO) at any time during the year.
- Evidence of Coverage (EOC) - The EOC explains your rights, benefits, and responsibilities as a member of Healthy Advantage (HMO). It also explains our responsibilities to you.
- Summary of Benefits (SOB) - The SOB outlines your plan premium, benefits, copayments and coinsurance for the current calendar year.
- LIS Chart - This table shows you what your monthly plan premium will be if you get extra help from Medicare in paying certain health care costs.
- Full Privacy - This notice describes how medical information about you may be used and disclosed. It also explains how you can get access to this information.
