835/EFT Test Procedures: Step 1 - Pre-testing Phase
Trading partners who have the ability to receive both their remittance advice and payment electronically must complete some initial set-up before UUHP can send the first 835 test file and pre-notification of the EFT (electronic funds transfer).
Please download the EFT Request Form and fax it to 801-587-6433 - attention EDI Coordinator. We will need a separate EFT Request form for each tax identification number you are submitting claims under - even if the bank account remains the same.
If you are not currently submitting 837 claims files to UUHP you will also need to download the Trading Partner Set-Up form and fax it to 801-587-6433 - attention EDI Coordinator.
After receiving your EFT Request form we will complete some initial system updates to capture this information for use during testing. During this time we suggest you identify and meet with your key billing office staff so they may have time to review their processes and make any needed changes.
